At Lewisham CCG we want to help local people live healthy, active and independent lives. As commissioners we aim to purchase NHS services that will deliver the highest quality, best value for money healthcare possible.
So that everyone gets the most from the services we commission, we work closely with other Lewisham health and social care providers to focus on:
- healthy lifestyles and choice, for example helping local people stop smoking, drink less alcohol, avoid obesity, and prevent cancer
- frail and vulnerable people, for example helping older people remain active and independent for as long as possible and making sure that more people can choose where to live out their final days
- long-term conditions, for example better support for people with chronic obstructive pulmonary disease (COPD), diabetes, cardiovascular disease, dementia, and other long-term conditions
- delivering services differently, for example looking at better ways to integrate health and social care to provide high-quality services best suited to the Lewisham community.
