​NHS Lewisham Clinical Commissioning Group and Lewisham and Greenwich NHS Trust has won a coveted Patient Safety Award


NHS Lewisham Clinical Commissioning Group (CCG) and Lewisham and Greenwich NHS Trust won the HSJ Patient Safety Awards Managing Long Term Conditions category and received a "highly commended" award for the Medicines Management category!


The results of the HSJ Patient Safety Awards 2015 were revealed last night at the ICC in Birmingham, where the healthcare industry elite came together for a night of recognition and celebration.


NHS Lewisham CCG and Lewisham and Greenwich NHS Trust won the Managing Long Term Conditions category. The Patient Safety Awards judging panel commented "This was a great presentation with an engaging patient story and a very positive message. It is cost effective and demonstrated excellent patient centred solutions."

The award recognises teams who have improved patient safety through effective management of long-term conditions. Of those shortlisted in this category, NHS Lewisham CCG impressed judges with its integrated and patient-centered approach to supporting patients with their medicines. The Lewisham Integrated Medicines Optimisation Service (LIMOS) has built a pathway for the referral of patients with medicines-related problems. Pharmaceutical care plans are agreed by patients and stakeholders alike. The judges were impressed that the team had created a pathway that was "cost-effective" and "patient-centered".
