Local Health Providers continue winning streak
An NHS service which helps Lewisham residents to manage their medication and remain independent has won a prestigious national award for the second time. The Lewisham Integrated Medicines Optimisation Service (LIMOS), commissioned by NHS Lewisham Clinical Commissioning Group, implemented by our partners Lewisham Council and Lewisham and Greenwich NHS Trust, was recognised in the Value in Healthcare Awards 2015. This follows on from their previous success in winning recognition at the Patient Safety Awards 2015.
Under the service, patients with long-term illnesses are helped to manage their own medicines and remain as independent as possible. Individual care plans are developed with the full involvement of patients, GPs, community pharmacists and social services. This is backed up by regular phone or home visits, to ensure that interventions continue to meet their needs and support them in remaining independent.
This innovative approach is helping some of our most vulnerable patients, including those who are housebound and who find it hard to use services. In the past 10 months 302 patients have been reviewed by the team. Of those, 111 possible accident and emergency attendances and 19 possible hospital admissions were avoided, and 150 unnecessary medicines stopped. It is estimated that for every £1 spent on delivering the service £2 is saved across health and social care.
The Value in Healthcare Awards judges said: “This project demonstrated clear clinical benefit and substantial financial benefits. It really shows what can be done in truly integrated care.”
