Lewisham LIMOS Shortlised for HSJ Award 

 

Lewisham Integrated Medical Optimisation Service otherwise known as ‘LIMOS’  has been shortlisted for HSJ (Health Service Journal) Award for Managing Long-term Conditions 2014.  The service has been developed and delivered in collaboration by NHS Lewisham Clinical Commissioning Group, Lewisham and Greenwich NHS Trust and the London Borough of Lewisham.

The Managing Long-Term Conditions Award rewards service innovators who put patients at the centre of care, involve them in planning new services, work with partners to deliver integrated care pathways, and can show better outcomes, more self-care by patients and fewer hospital admissions.

'LIMOS’ supports patients with long term conditions to manage their own medicines to enable them to stay in their own home for as long as possible.   It was shortlisted as one of 11 finalists from over 200 applications.  The team were invited to present to the HSJ judges panel on 9 October as part of the application process

Martin Wilkinson, Chief Officer at NHS Lewisham Clinical Commissioning Group said: “We are so proud of the LIMOS service and very excited at the prospect of winning a HSJ award.  It is run by a small team of specialist pharmacists and pharmacy technicians who work across primary, secondary and social care with consultant input – it is a great example of collaborative working.  Whatever the outcome on 19th, it is a winner for me!  Good luck and well done to the Lewisham team.”

The winner will be announced at the HSJ Awards 2014 on 19 November.
