Diabetes programme shortlisted for HealthCare Transformation award


NHS Lewisham CCG's Innovation in Diabetes care has been nominated for an award at The Healthcare Transformation Awards hosted by NHS Clinical Commissioners and Health + Care.

The new programme of care brought together diabetes and mental health treatment for a patient group that is growing in Lewisham.  The intention is to provide support and education for mental health carers to give them a better understanding of diabetes and feel better enabled to support the person they are caring for  their clients to access diabetes care and help prevent the long term complications of diabetes. Nine diabetes care processes were implemented including training from a diabetes specialist nurse, integrated clinics and education programme with cookery demonstrations.

Martin Wilkinson, Chief Officer at NHS Lewisham CCG said, "Managing Diabetes better is a national priority, and important for us locally as we know that over 15,000 adults have been diagnosed with Diabetes in Lewisham last year, that's around 6% of our population.  Developing new ways to help patients manage this condition irrespective of other circumstances or long term conditions they may experience is important and we are proud that our local innovations and excited to be nominated for a Healthcare Transformation Award".

The Healthcare Transformation Awards hosted by NHS Clinical Commissioners and Health + Care will take place 29 June 2016 at The Crown Plaza, London Docklands.  ​
